2. Choose Multiple Sites: You can select multiple program sites on one application!

3. Eligibility Ranking: Families are ranked based on family size, income, and need according to state regulations.

To enroll, you’ll need:

• Child’s birth certificate (and for all siblings under 18)

• Physician’s Report

• Immunization Records

• Proof of Address

• (Upon request) One month proof of income

• Turnbull Child Development Center (Full-time: 8:30 AM–4:30 PM)

• LEAD Elementary School (Full-time: 8:30 AM–4:30 PM / Part-time: 8:30 AM–12:00 PM)

• Sunnybrae Elementary School (Full-time: 8:30 AM–4:30 PM)

• Laurel Elementary School (Full-time: 8:30 AM–4:30 PM)

• San Mateo Park (Full-time: 8:30 AM–4:30 PM)

• Foster City (Full-time: 8:30 AM–3:00 PM)
