Required Registration Documents

The following documents and forms are required to register in the San Mateo-Foster City School District. The office staff of the school of assignment will review and verify the required registration documents. Documents can be uploaded in the online registration form or can be given directly to the school office staff either by email or in-person at the school office.

  • Proof of Student’s Age: Student’s certified birth certificate, passport, duly attested baptismal certificate, certified statement by the local registrar or county recorder certifying the date of birth. When none of the foregoing documents is obtainable, an affidavit of the parent/guardian.
  • Child’s original, up-to-date immunization record
  • Parent/Guardian current photo identification: current California State Driver License, current California State ID Card or Valid Passport or Consulate Issued Picture ID, or other photo identification.
  • Proof of District Residency — Provide one original, current proof of district residency from the list below:
    • Declaration of District Residency
    • Current property tax bill
    • Current rental/lease agreement listing the parent/guardian name, child name(s), and name of property manager/owner and telephone number
    • Current homeowner’s or renter’s insurance policy
    • State or federal tax return with a W-2 attached (please remove social security number and income information from these documents before submitting)
    • Payroll/check stub with name and address
    • Voter Registration
    • Current utility bill (e.g. PG&E, landline telephone, water, garbage, or cable TV).
    • Correspondence with a government agency.
    • Bill or correspondence must be dated within the past 60 days.

District Residency: Students must be physically living with their parent/guardian within district boundaries at the time of registration (AR 5111/Education Code 48200). Ownership of real property within the district does not in and of itself constitute residency.

  • Optional (formerly required) Child’s original Report of Health Examination for School Entry submitted at the time of the student's initial entry into school. DHCS Report of Health Examination for School Entry
  • Kindergarten Oral Health Assessment (KOHA). This form is not required for registration, but required of TK and Kindergarten students who attend public TK or Kindergarten in the state of California. The KOHA was passed into law by the California state legislature.
    • Transitional Kindergarten or Kindergarten - Assembly Bill 2630 went into effect January 1, 2025. It clarifies that the kindergarten oral health screening is required once during either transitional kindergarten or kindergarten for children in public schools.
    • Any student in first grade in public school, only if they did not attend public school Transitional Kindergarten or Kindergarten - Some children do not attend public school transitional kindergarten or kindergarten, and their first year in public school is first grade. For these first grade students only–not all public school first graders–the oral health assessment is required. 
    • Due dates- The forms for all Kindergartners and only those 1st graders in public school for their first year should ideally be turned into the school within the first half of the school year. However, the final due date for parents/caregivers to turn in a completed KOHA or Waiver form is May 31st (or by the end of the school year for those schools that get out before May 31st).
