6th Grade Verification for Current 5th Grade Students

HELLO FAMILIES OF CURRENT 5TH GRADE SMFCSD STUDENTS!

Every year, current 5th Grade Families complete the 6th Grade Verification Process for the upcoming school year.  This process provides District middle schools and K-8 schools with important information to assist them in planning for the upcoming school year.  This process will take place between April 5 -14, 2023.

Your child’s middle school of assignment will email you a link to an online 2023-2024 6th Grade Verification Process Questionnaire during the week of April 3, 2023.  The 2023-2024 6th Grade Verification Process Questionnaire will ask you about your enrollment plans for next school year.  You will also be asked to provide one (1) current proof of district residency so that the middle school may verify your current address of residence.  The list of accepted proofs of residence documents is below.

You have two ways you can complete the 6th Grade Verification Process, select one option:

Option 1: Online Form  Your child’s middle school of assignment will send you a link to the online 6th Grade Verification Process questionnaire.  You can complete the questionnaire and upload the required current proof of district residency.

Option 2: Paper Form  Your child’s middle school is offering drop-in office hours.  You can complete a paper version of the questionnaire and present the current proof of district residency in-person.  School offices will have paper copies of the form available, a PDF of the paper form is also attached.  The schedule of drop-in office hours is below.

NOTE: if you received and accepted a transfer to another middle school for 6th grade, you will complete this process at the approved transfer school.

Accepted Proofs of District Residency - Provide one (1) document from the list below:

  • Payroll/check stub with name and address

  • Voter Registration

  • Declaration of District Residency (click for the link)

  • Current property tax bill

  • Current rental/lease agreement listing the parent/guardian name, child name(s), and name of property manager/owner and telephone number

  • Current homeowner’s or renter’s insurance policy

  • State or federal tax return with a W-2 attached (please remove social security number and income information from these documents before submitting)

  • Current utility bill (e.g. PG&E, landline telephone, water, garbage, or cable TV)

  • Correspondence with a government agency.

    • Bill and correspondence  must be dated within the past 60 days.
